HR Form Question

Main information about the form questions registered in the system is stored in the Form Question (hrm_from_question) table.

Roles required:

  • Read – hrm_supervisor, hrm_form_manager, hrm_form_launch_manager, hrm_admin, and employees following the form.
  • Create, update, delete – hrm_form_manager, hrm_admin.

Add a question


You can add a question to an HR form in the Draft state. To do so, complete the following steps:

  1. In the agent interface, navigate to Human Resource ManagementForms and surveysForms and open the required form.
  2. Click Add question in the upper-left corner and fill in the fields in the modal window that opens.
  3. Click Add.

Add a question to the form modal window fields

The set of fields in the Add a question to the form modal window depends on the selected form category. The field set is the same for Questionnaire and Survey, and is different for Test.

Response formatYSelect a question format. Available options:
  • Brief constructed is a short text answer provided by the respondent.
  • Constructed is a detailed text answer provided by the respondent.
  • Single select multiple choice presents a list of response options where only one option provided by the question author is correct.
  • Multi-select multiple choice presents a list of response options where several options provided by the question author are correct.
  • File upload is an answer uploaded as a file by the respondent.
OrderYSpecify the question order on the form.
MandatoryNSelect the checkbox to make the question mandatory. The checkbox is cleared by default.
