HR Form Question Response Option

Main information about the response options for form questions is stored in the Form Response Option (hrm_form_response_option) table. Form response options are available only for questions with the Single select multiple choice and Multi-select multiple choice response formats.

Add a response option to a new question record

To add a response option when you create a new question record, complete the following steps:

1. Create an HR form.
2. Add a question to the HR form.
3. In the page that opens, click Add response option in the upper-left corner.
4. In the modal window that opens, complete the required fields and click Add.

Field	Mandatory	Description
Response option	Y	Specify a response option.
Order	Y	Specify the order of the response option.
Correct option	Y/N	Select the checkbox if this response option is correct. At least one response option must be correct for a question. The field is available if Test is selected as the HR form category, and the response format is Single select multiple choice or Multi-select multiple choice.
Score per option	Y	Specify the score to be obtained by the respondent if they select this option. The field is displayed if the Correct option checkbox is selected.

Add a response option to an existing question record

To add a response option to an existing question record, complete the following steps:

1. In the agent interface, navigate to Human Resource Management → Forms and Surveys → Forms and open the HR form record with a question to which you need to add a response option.
2. Switch to the Questions tab in the related lists area and a form record.
3. Click Add response option in the upper-left corner and fill in the fields.
4. Click Save or Save and exit to apply the changes.

Field	Mandatory	Description
Question	Y	The field is automatically filled with the question the response option is related to.
Response option	Y	Specify a response option.
Correct option	Y/N	Select the checkbox if this response option is correct. At least one response option must be correct for a question. The field is available if Test is selected as the HR form category, and the response format is Single select multiple choice or Multi-select multiple choice.
Score per option	Y	Specify the score to be obtained by the respondent if they select this option. The field is displayed if the Correct option checkbox is selected.
Order	Y	Specify the order of the response option.
